Long Island State Veterans Home

The LISVH is a 350 bed skilled nursing facility located on 25 wooded acres on the campus of Stony Brook University. An Adult Day Health Care program that operates 6 days per week (Monday through Saturday) is also offered.

The LISVH is operated by the Health Sciences Center of Stony Brook University and continues to be one of the only skilled nursing centers and adult day health care programs in the country that is fully integrated into the health and educational mission of a major teaching and research university. The programs at the LISVH are licensed by the New York State Department of Health and the U.S. Department of Veterans Affairs.

The LISVH accepts honorably discharged veterans from all branches of the United States Armed Forces. Admission to the Long Island State Veterans Home and the Adult Day Health Care Program is open to all veterans regardless of period of service (war or peace time service), service connected disability or location in which you served our nation. The LISVH also accepts the spouse or widow of qualifying veterans, as well as Gold Star Parents who require skilled nursing or adult day health care.

In order to process a request for admission,
the following documents are required:

LISVH Application (4 pages completed and signed). This application can be completed online, faxed or mailed to the Admissions Office at the LISVH.
Copy of Veteran’s Discharge papers or other proof of VA status
Copy of both sides of insurance cards including:
Medicare Card
Medicare Part D Card
Supplemental Insurance Card
Prescription Plan Card
Copy of legal documents such as Power of Attorney (POA), Health Care Proxy, Living Will, etc.
Patient Review Instrument (PRI) and Screen
The Patient Review Instrument (PRI) and Screen is a New York State required assessment of the applicant’s medical needs

Financial Advantages

Choosing a New York State Veterans Home for skilled nursing care can provide a veteran with significant financial savings. There are a number of financial benefits and incentives available for Veterans utilizing NYS Veterans homes.

The Federal Per Diem Payment program will assist any honorable discharge Veteran who is paying out-of-pocket for their skilled nursing care if they utilize a NYS Veterans Home. NYS Veteran Homes will apply the Federal Per Diem payment as a credit to the veteran’s monthly statement. This per diem will reduce the daily rate for any veteran who is paying privately. The veteran will save over $28,000 per year by taking advantage of this per diem benefit.
The Non-Service Connected Disability Pension Benefit- Aid and Attendance-is a special exemption for single Veterans on Medicaid in State Veterans Homes. This benefit is a needs-based pension benefit for low-income Veterans who are disabled and require daily assistance in bathing, dressing, meal preparation, medication monitoring or other various daily activities. The Aid and Attendance is based on a disability not related to an injury or illness sustained during or as a result of military service and a VA disability rating is not required. To qualify for this benefit, the veteran needs to be an honorably discharged veteran who served at least ninety days of “wartime service”.
The VA pays full cost of skilled nursing care at State Veterans Homes for Veterans with 70% or greater service connected disability. The Department of Veterans Affairs has established a rating schedule to aid in the evaluation of disabilities resulting from disease or injury during or as a result of military service. There are no out-of-pocket costs charged to the Veteran. There is no means test or income limitation associated with this benefit. In addition, the veteran is entitled to keep all social security, pensions, or other income that would normally be used to offset his or her costs of long term care.
