Who is Eligible for Our Program

At the Long Island State Veterans Home, we believe that an important part of providing service to our nation’s veterans includes offering the best possible health care to a veteran’s family as well. For this reason, our adult day care program specializes in the care and treatment of both veterans and their loved ones including, SPOUSES, WIDOWS and Gold Star Parents.

Transportation

We provide door-to-door transportation to and from our adult day care program. The cost of transportation is included in our daily rate for service.

What is the Admission Process?

Once you have met the program’s eligibility criteria, we will provide a free comprehensive nursing and social work consultation. We will assess your clinical needs and determine how our services can benefit you and help you reach your goals. Our clinical team will then coordinate your care needs with your family physician to ensure that you receive the services that best suit your needs.
